Focusing on the new theme, from September 3 to 6, 2024, the 26th National Stomatology Academic Conference of the Chinese Stomatological Association invited well-known experts and scholars in the field of stomatology at home and abroad to organically integrate the latest progress of intelligent technology in the field of stomatology into the academic content of the annual meeting.

Let's review the highlights of the academic venue of this annual meeting.

On the morning of September 4th, the "FDI-CSA Symposium on Stomatology - Digital Implantology" was held by Dr. Murphy, Head of Continuing Education, Asia Pacific, FDI. Simon Kam Yuen HO and Professor Li Dehua from the Stomatological Hospital of the Air Force Military Medical University co-chaired the meeting, which brought together Chinese and foreign dental industry pioneers to discuss the development of digitalization in the dental industry yesterday, today and tomorrow.

Dr. Tan Sock Hooi introduced the cutting-edge technology and latest trends of digital implantology. Digitally guided implant surgery has many advantages, on the one hand, it can achieve the purpose of minimally invasive, and on the other hand, it is very beneficial for patients with systemic diseases, the elderly, and people who are afraid of invasive surgery. At the same time, the accuracy of digitally guided implant surgery is also limited by some factors, which is also a difficulty that we need to overcome in the future.
Prof. LEE Sung Bok shared her research on how full-mouth All-on-X fixed implant dentures can achieve immediate functional restoration. The main advantage of immediate implant restoration is that it reduces the need for transitional dentures, which are not so convenient for patients; In addition, immediate implants can also provide space and time for soft tissue healing, which can help restore the preoperative soft tissue structure.
Dr. Tan Fook Seng Dental Clinic from Hong Kong, China Brian Fuk-Shing Chan gave a detailed introduction to the complete digital workflow of All-on-X, showing the latest digital design process for full arch implant treatment by showing the treatment process of several cases, such as recording the exact implant position, scanning the implant site marker in the mouth, and temporarily customizing the bite pad, etc., which made us feel confident in the development of the digital workflow. On the afternoon of September 4th, the special discussion on endodontics was planned and presided over by Professor Yue Lin, vice president and secretary general of the Chinese Stomatological Association, with the theme of "tracing the roots, understanding the micro and the micro", focusing on "the accidents in the development of the root bring clinical cognition and new challenges", and invited more than 20 multidisciplinary authoritative experts in oral histopathology, pediatric stomatology, periodontology, orthodontics, endodontics and other closely related multidisciplinary authoritative experts, focusing on three key issues: 1. Confusion of apical localization in root development, 2. Excessive proliferation of the epithelial root sheath in root development (root inversion problem), 3. Insufficient proliferation of epithelial root sheath in root development (C-shaped root canal and bovine dental problems), a review report and review summary were carried out.

After listening to the evidence-based sharing and analysis discussions of experts, most of the doctors in the audience agreed with the following three views:1. The position of the root apex is not fixed during root development; 2. The deformed root groove belongs to the root inversion; 3. The pathogenesis between C-shaped root canals and bovine tooth disease may be involved. (CMT Jiajing)

Oral pain management

On the afternoon of September 4, Professor Guo Chuanyao, President of the Chinese Stomatological Association, delivered a speech, and was co-chaired by Huang Yulei, Deputy Chief Physician of the Affiliated Hospital of Stomatology of Sun Yat-sen University, and Tao Jiang, Chief Physician of the Ninth People's Hospital Affiliated to Shanghai Jiao Tong University School of Medicine.

At the meeting, Professor Yu Dongsheng from the Affiliated Stomatological Hospital of Sun Yat-sen University shared a new understanding of oral pain, saying that more than 50% of oral emergency patients are because of pain, and in severe cases, it even affects daily life, among which iatrogenic pain has attracted much attention. Oral treatment of advanced analgesia can prevent the transmission and conduction of peripheral injury impulses to the center and avoid pain sensitization, and peripheral opioids (such as ibuprofen, nimesulide, etotrixib, etc.) can be used for advanced analgesia. Dentists' understanding of pain needs to be further updated, and pain control in the whole diagnosis and treatment cycle deserves our attention.

Associate Professor Fang Ming of the Stomatological Hospital of the Air Force Military Medical University gave a wonderful speech on the new theory and method of dental bonding. The development of oral bonding technology is comprehensively analyzed from five directions: theoretical update, material iteration, technological progress, consensus establishment and future direction. Acid etching-rinsing mode is the best bonding strategy for tooth enamel bonding, which requires independent acid etching and rinsing steps to completely remove the stain layer and has more than 20 years of long-term clinical follow-up. The disadvantage is that phosphoric acid is used for dentin acid etching too strongly, resulting in excessive collagen exposure, requiring resin to penetrate deep into the demineralization area, and the area with insufficient penetration is prone to leakage and enzymatic hydrolysis, and only secondary chemical bonds can be formed during the bonding process. For self-etching bonding, it is more suitable for dentin bonding, does not require independent acid etching steps, and belongs to partial demineralization, less collagen exposure, reduced enzymatic hydrolysis risk, because acid etching and resin penetration are synchronized, so there is less infiltration, and self-etching is to dissolve the modified stain layer, rather than remove the stain layer, so dentin is less sensitive; The disadvantage is that the functional monomer is sensitive to hydrolysis, and it is recommended to use it with selective acid etching of tooth enamel, and the long-term clinical research of the self-etching system in a single bottle is limited. In this seminar, the chairman of the Orthodontic Committee of the Chinese Stomatological Association and Professor Fang Bing of the Ninth People's Hospital affiliated to Shanghai Jiao Tong University School of Medicine looked at periodontal problems from the perspective of orthodontists, if orthodontic treatment has a positive impact on periodontal health, then it is worth carrying out, periodontal orthodontics and maintenance should follow the choice of lifelong maintenance, and to do a good job in doctor-patient communication, physicians should consider the cost performance of tooth extraction restoration and orthodontic treatment for patients.

Professor Pan Yaping from the Stomatological Hospital of China Medical University raised a question: Is there an optimal orthodontic time for periodontal-orthodontic combined treatment? Patients with reduced periodontal tissue but stable periodontal status receive orthodontic tooth movement with no additional or minimal loss of attachment. However, the optimal time interval between orthodontic and periodontal treatment is unknown. After reviewing a lot of literature, it has been found that early orthodontic tooth movement has a "stimulating" effect on regenerative results, and orthodontic treatment can be started much earlier (immediately up to 3 months after regenerative surgery). Orthodontic tooth movement at 3 months after regenerative surgery treatment does not appear to have a negative impact on treatment outcomes, greatly reducing the overall treatment time for patients.

Associate Professor Ding Peihui from the Hospital of Stomatology Affiliated to Zhejiang University School of Medicine shared the "Clinical Minutes of the 2024 EFP Periodontal Orthodontic Combined Therapy Master Conference" at the meeting, with a total of 8 topics, and experts and scholars from Switzerland, Japan and several countries at home and abroad expressed their views and research on orthodontic and periodontal combined treatment.

Dr. Xiaoxiao Dong, Chief Physician of Beijing United Family Hospital and Clinic, and Attending Physician Cheng Zhang shared their new ideas on periodontal orthodontic treatment for periodontitis patients, starting from the questions of "why periodontitis patients are also an important group of adult orthodontics", "can patients with severe periodontitis undergo orthodontics", "how to perform orthodontic treatment for periodontal patients", "characteristics of orthodontic appliances for periodontal patients", and "long-term prognosis". Won unanimous praise from everyone. (CMT Orange Orange)

Oral clinical standard medication

640 (11).jpg

In the seminar on clinical standardized medication in dental clinics, Du Hongming, deputy chief physician of the Affiliated Hospital of Stomatology of Nanjing Medical University, explained the selection and use of oral local anesthetic drugs, commonly used anti-infective drugs and analgesic drugs. For local anesthetic drugs, the following points need to be noted: 1. Local anesthetic operation and application of local anesthetic drugs should be carried out under the condition of sufficient first-aid facilities; 2. The patient's allergy history, disease history and previous medication history should be inquired in detail before taking the drug, and the appropriate local anesthetic should be selected to reduce the patient's anxiety and fear, and the patient should accept and cooperate with anesthesia with the informed consent of the patient; 3. Clinicians should be familiar with the pharmacological effects, clinical dosage, adverse reactions and prevention and treatment measures of the local anesthetic used; 4. The treatment of toxic reactions should be based on prevention, grasp the drug concentration and the maximum allowable dose at one time, and adopt the method of divided small dose injection (it is necessary to pay attention to the speed of anesthetic bolus, and there is no blood in the withdrawal); 5. Individual differences should be paid attention to during the medication, such as elderly patients with organ degeneration, children, pregnant women, liver and kidney insufficiency and other patients should be appropriately reduced, and the minimum dose and minimum effective concentration should be used under the premise of ensuring the effect of local anesthesia; 6. During and after the operation, the patient's vital signs should be paid close attention to, and effective prevention and control measures should be taken in case of adverse occurrences, and rescue should be organized immediately for the critically ill; 7. Accurately grasp the dosage and maximum amount of local anesthetic, and grasp the timing of administration in time, in order to achieve maximum comfortable medical treatment. (CMT Kaede)

High-quality development of private dentistry

640 (12).jpg

In the seminar on the high-quality development and management of private dentistry, He Zhou, former chairman of the Stomatological Private Medical Branch of the Chinese Stomatological Association and president of Beijing Huajing Dental Department, shared that the growth in the era of low profits, opportunities that are not easy to find and mistakes that are easy to ignore are the two key elements related to the survival of the clinic. The decomposition of details, the fine management of routine medical care, and the continuous pursuit of quality and efficiency are the development opportunities of dental private clinics, and supplementation, collaboration and differentiation should be the positioning of current dental private clinics. The triadistic concept of medical technology in private clinics includes three attributes: treatment, service and operation. Everything is visualized, the patient side is conducive to experience and communication, and the doctor side can realize diagnosis and communication.

Chen Xuefeng, chairman-elect of the Private Dental Branch of the Chinese Stomatological Association and president of Huai'an Yuehong Dental Clinic, shared his thoughts and prospects on the high-quality development of private dentistry. Among them, it emphasizes the importance of improving the experience of medical treatment and enhancing the patient experience for private oral healthcare. (CMT Kaede)
